常规超声积分法鉴别乳腺良、恶性肿物

Differential Diagnosis of Benign and Malignant Breast Masses by Conventional Ultrasound#br# Integral Method

摘要: 目的:探讨乳腺肿物的常规超声(ultrasound,US)恶性征象,评估US 积分法鉴别乳腺良、恶性肿物的诊断效能。
方法:回顾分析120 例经病理证实的乳腺良、恶性肿物的US 图像,对US 恶性征象赋值求积分,应用受试者工作特征曲线
(ROC)分析积分法的最佳临界值及诊断效能;采用Kappa一致性检验评估US积分法与病理诊断的一致性。结果:二维形态
特征、Adler血流分级及年龄在良、恶性组间差异有统计学意义(P <0.01);将积分值7作为US积分法的最佳临界值,诊断效
能最佳(敏感度、特异度、准确率、约登指数分别为92.20%、92.80%、92.50%、0.850);US积分法与病理诊断一致性检验的Kappa
值为0.847(P <0.01)。结论:US积分法在乳腺良、恶性肿物鉴别诊断中具有较高的诊断价值,其与病理诊断的一致性高。

Abstract: Objective: To explore the malignant signs of breast masses by conventional ultrasound (US) and to evaluate the
diagnostic efficiency of US integral method in differentiating benign and malignant breast masses. Methods: The US images of 120
cases of benign and malignant breast masses confirmed by pathology were analyzed retrospectively, and the value of US malignant signs
were assigned and integrated. The receiver operating characteristic curve(ROC)was applied to analyze the cut-off value of integration
method and its diagnostic efficiency. Kappa-test was used to evaluate the consistency between US integral method and pathological
diagnosis. Results: Two-dimensional morphological characteristics, Adler blood flow classification and age were significantly different
between benign and malignant groups(P <0.01), which were malignant signs of US. When taking the score 7 as the best cut-off value
of US integral method, the diagnostic efficiency is the best(sensitivity, specificity, accuracy and Youden index were 92.20%, 92.80%,
92.50% and 0.850 respectively); the Kappa value between US integral method and pathological diagnosis was 0.847(P < 0.01).
Conclusion: US integral method has high diagnostic value in differential diagnosis of benign and malignant breast masses, and its
consistency with pathological diagnosis is high.
